[Cable] allow me to share a curious tale

So I'm not currently a Teksavvy user, but I was up until late June of 2012. I ended up switching back from Teksavvy to Shaw because I needed a faster upload and at the time and Teksavvy was only offering the 25/2.5 cable package. Off my cancellation request went, it came back confirmed; the day of cancellation came and went and service was switched over (or so I thought.) Fast forward to now, I get a call from Teksavvy indicating that there's a problem and it turns out my connection *wasn't* cancelled and now Shaw was looking for Teksavvy to pay up and consequently I may be on the hook to pay up for what might have been free internet usage for the past 1.5 years. Now, I haven't been using the Teksavvy internet at all since the day of the switch; actually I unplugged that modem a few days before cancellation day because it overlapped with a new install of Shaw internet which I had installed before the other one was cancelled. After confirming with the Teksavvy agent that I have in fact been using and paying Shaw since the cancellation (I had the previous bills to prove it,) apparently Shaw came back and claimed I was still a Teksavvy customer, according to the TSI agent. I know for a fact I haven't been using the Teksavvy service as I have been using a package that wasn't even available on Teksavvy until a few months ago, and on top of that I had a Shaw IP and was routed through the Shaw gateways (not TSI's.) Anyways, with my last communications with Teksavvy - who are now apparently my ISP, somehow, even though I still don't have a TSI IP, or are routed through their gateway, or even using their modem - I now have a new cancellation date setup, and have been told I don't owe any money (not that I would have paid anyways...) I'm sure this doesn't happen often, or at least I haven't heard of it mentioned here often, and it seems like there should be guards in place to prevent this kind of thing happening, but has anyone ever heard of or experienced any issue like this? My only suggestion to Teksavvy here would be to be a little more diligent in handling these cancellations by either getting the provider (in this case, Shaw) to confirm the cancellation, or something to that effect - basically anything other than just fire off the request and forget as it seemed to be in this case, because as I've come to find out this could lead to potential issues down the road. TL;DR - Cancelled Teksavvy cable service (provided by Shaw) to switch to Shaw, service wasn't actually cancelled by Shaw and they asked to get paid for non-cancelled service. Old customer contacted and after some back and forth things (hopefully) got resolved.
